ITEM 5 RELOCATE WASTE ROCK <br /> During mining activities, the operator illegally moved waste rock from the Freda Mine site <br /> permit boundary,up the access road and off of private property onto USFS managed lands. The <br /> waste rock located 600 feet up the continuation of the access road will be relocated back onto the <br /> Tornado Claim and back onto the permit boundary area to the removed mill site area. It is <br /> estimated that 500 cubic yards of waste rock are located off the property that will be relocated <br /> 600 linear feet back to the property. The Contractor is required to pick the waste rock up to <br /> move the material back on site,most likely with a front end loader. <br /> Measurement and Payment <br /> Payment for this task will be made at the unit-price established in Item 5 of the Bid <br /> Schedule. Measurement for payment will be made per cubic yard of material relocated <br /> back onto the property. Payment for this task will be upon completion of installation and <br /> upon approval of the PROJECT MANAGER. Such payment shall be considered full <br /> compensation for all labor,materials, equipment and all other items necessary and <br /> incidental to the completion of Bid Item. <br /> ITEM 6 PROVIDE LIMESTONE CRUSHER FINES <br /> Under this Bid Item,the CONTRACTOR will procure and deliver 5 cubic yards of fine-grained <br /> limestone crusher fines from the Monarch Quarry, or equivalent as approved by the PROJECT <br /> MANAGER, for incorporation into the waste rock that is hauled back onto the permit boundary. <br /> The limestone crusher fines will be used to amend the waste rock because it has the highest acid <br /> generating potential of all three wastes at 5.94 TCaCO3/KT of waste. The tarp covered ore pile <br /> has no acid generating potential and the mill tailings have only 0.90 TCaCO3/KT of tailings. <br /> The limestone fines will curtail acid formation and heavy metals leaching. Limestone must be at <br /> least 85% calcium carbonate equivalent or greater. <br /> This Bid Item includes the delivery of the limestone crusher fines in one cubic yard(ICY) super <br /> sacks to the Freda Mine site. Delivery of the limestone in haul trucks will be permitted to the <br /> Ophir Pass Bridge (Burro Bridge) which is 0.24 miles off of Hwy 550. The limestone crusher <br /> fines will then need to be transported up to the mine site,which is 1.4 miles to the Freda Mine <br /> Access Road. The Freda Mine access road is 0.9 miles long. <br /> Measurement and Payment <br /> Payment for this task will be made at the unit price established in Item 6 of the Bid <br /> Schedule. Measurement for payment will be made per cubic yard of fine-grained <br /> limestone procured and delivered. Payment for this task will be upon completion of <br /> installation and upon approval of the PROJECT MANAGER. Such payment shall be <br /> considered full compensation for all labor, materials, equipment and all other items <br /> necessary and incidental to the completion of the Bid Item. <br /> Page 14 of 17 <br />
