PERMIT 4: M-2018-011 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: December 8, 2021 <br />INSPECTION TOPIC: Signs & Markers <br />PROBLEM/POSSIBLE VIOLATION: Problem: The affected area boundary markers are missing or incorrectly <br />placed. This is a problem for failure to maintain boundary markers around the affected area as required by <br />Section 3.1.12(2) of the rule. <br />CORRECTIVE ACTIONS: The operator shall conduct a survey and replace the boundary markers in the correct <br />location(s). The operator shall provide proof to the Division that this has been done by the corrective action <br />date. <br />CORRECTIVE ACTION DUE DATE: 1/21/22 <br />OBSERVATIONS <br />This was a routine monitoring inspection conducted by Eric Scott of DRMS, accompanied by permittee <br />representative Mike Volosin of Ames Construction. The site is a 12 acre 111c permit with a $51,014 <br />reclamation bond. The approved reclamation plan for the site is for an irrigation recharge pond (unlined but <br />sloped excavation) surrounded by the approved reclamation vegetation consisting primarily of rangeland <br />grasses. The site identification sign was present (photo 1) but permit boundary markers were missing. The <br />permit boundary markers will need to be replaced by the corrective action date given, and maintained until <br />the permit is released. <br />Conditions at the site have not changed since the inspection in late 2019. Although mining ended at the site in <br />mid-2018, no reclamation vegetation has been established in the affected area outside of the recharge pond. <br />The existing on -site vegetation consists of entirely of weedy species such as Russian thistle (photos 2-5). <br />As stated on the first page of this report, revegetation efforts will need to be restarted and properly <br />maintained so site can be released within statutory timeframe - within 5 years of end of mining. The entire <br />permit area outside of the recharge pond area will need to be regraded, reseeded, mulched, and the approved <br />reclamation vegetation established before the remaining permit can be released.
