The <br />inspection was conducted, images analyzed and the report prepared by me, Robin Reilley of the DRMS. Miranda <br />Kawcak mine representative was present throughout the inspection. No hydrologic or air quality issues were <br />detected at the time of inspection. There was a light rain falling and no breeze. Temperatures were in the <br />mid 30 degrees F. <br />OUTSTANDING MAINTENANCE ITEMS: <br />A replacement mine identification sign is required at the main entrance from the County Road. <br />INSPECTION. - <br />GENERAL MINE PLAN COMPLIANCE: <br />Reclamation is complete and executed as permitted. Approximately 106.2 acres of Phase I released acres remain <br />for bond release. These acres comprise the main haul road, Pond 6 and the slide area above; Ponds 15 and 16 and <br />slide area on the east side of the site. Year nine vegetation sampling of the above mentioned areas is projected for <br />2022. Reclamation appears stable with well established vegetation. <br />SIGNS and MARKERS — Rule 4.02 <br />o The mine site identification signs observed at permit boundary along main haul road were intact, <br />displaying all required information: mine name, operator name, address, phone number, DRMS permit <br />number and contact information. <br />o Permit boundary markers on the main haul road were intact and adequately spaced. <br />ROADS Rule 4.03 <br />Construction 4.03.1(3), 4.03.2(3); Drainage 4.03.1(4), 4.03.2(4); Surfacing and Maintenance 4.03.1(5) and (6) <br />4.03.2(5) and (6). <br />o Roads were wet, muddy and slippery due to rain causing only partial access to the site due to steepness <br />and the likelihood of causing road damage and rutting. <br />o The main haul road was stable and in good condition, although saturated with small amounts of standing <br />water in places. <br />o There was rutting on the access road down to Pond 16. <br />Number of Partial Inspection this Fiscal Year: 5 <br />Number of Complete Inspections this Fiscal Year: 2 <br />Page 2 of 5 <br />
