Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation,Mining and <br /> Safety as part of the normal monitoring program for permitted sites. Frank Ferns of Elk Ridge Mining and <br /> Reclamation accompanied the inspection and represented the Operator. The site is in Final Reclamation and was <br /> not active the day of the inspection. the weather was clear and dry,Five photos accompanty this report to illustrate <br /> the observed site conditions <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> Both Ponds located within the Permit Area were observed during the inspection. Pond 001, shown in <br /> Photo One,was stable at the time of the inspection and in good condition. The side slopes of the pond showed no <br /> signs of settling, slumping or erosion and are well vegetated. The floor of the pond contained no standing water <br /> and the vegetative cover suggests that no water has been present in the pond recently. Both inlet structures to <br /> Pond 001 were observed to be in good condition and stable at the time of the inspection. <br /> Pond 002 located towards the East end of the Permitted area was also observed to be in good condition and can be <br /> seen in Photo Two and Three. The banks and side slopes of the pond appeared stable and no evidence of erosion <br /> was noted. No standing water was present during the inspection, and similar to Pond 001,the condition of the <br /> vegetative cover suggests no water has been present recently. Both inlet structures are in good condition and no <br /> holes or tears in the liners was noted. The inlets are also visible in Photos Two and Three. <br /> No fuels,oils or hazardous materials are stored on site. <br /> TOPSOIL—Rule 4.06 <br /> Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br /> All overburden and topsoil stockpiles were observed during this inspection. The Overburden stockpiles <br /> are located near both Pond 001 and 002.
