Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation,Mining and <br /> Safety as part of the normal monitoring program for permitted sites. Wade Wymore accompanied the inspection <br /> and represented the Operator. Normal activies were tkaing place at the mine during the inspection. at the King II <br /> area,the stackers and loaders were active and trucks were bineg loaded at the time of the inspection. No activities <br /> were being conducted at the King I area. The weather was clear but damp from a precipitation event on the night <br /> before.Nine Photos accompany this report to illustrate the current site conditions. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> The site received a significant precipitation event the night before the inspection. As a result the storm <br /> water conveyances and management systems were inspected at both the King I and King II facilities. <br /> King I facility: The north Clear Water Ditch was observed and had minimal standing water. The ditch appears to <br /> have functioned as designed in diverting and containing upland flow. Despite the standing water in places of the <br /> ditch,the vegetation did not suggest significant flow. The south diversion ditch at the edge of the upper refuse pile <br /> was observed to be in good condition. A few spots showed minor erosion within the ditch however this is not <br /> considered a problem at this time. The erosional features observed during the previous inspection on the upper <br /> refuse pile have been addressed and not considered a further problem. The upper refuse pile can be seen in Photo <br /> One. The storm water conveyances and culverts along the access road leading from the lower refuse pile to the <br /> upper refuse pile appeared to be functioning as designed and no evidence of significant erosion was noted. An <br /> example of the access road can be seen in Photo Two. The lower refuse pile exhibits positive drainage and no <br /> impounded storm water was noted. A significant erosional feature was identified near the access point of the <br /> lower refuse pile. The flow from the erosion was contained within the affected area and directed to the lower <br /> sediment ponds accordingly. The feature, shown in Photo Three,is not considered a problem at this time. <br /> Converstaions with the Operator indicate that repairs would be made by the end of the day. <br /> Number of Partial Inspection this Fiscal Year: 2 <br /> Number of Complete Inspections this Fiscal Year: 1 <br /> Page 2 of 8 <br />
