0 S <br /> Environmental &- Disaster Restoration, Inc. <br /> Services Inspection Address: 7911 Upper River Road <br /> Flood and Water Woody Creek, CO 81656 <br /> Damage Restoration <br /> Asbestos Testing and To whom it may concern, <br /> Abatement <br /> Mold Remediation and On 5/20/21 ECOS Environmental and Disaster Restoration Inc. was contacted by Jim <br /> Removal Doody with Elam gravel pit located at the above listed inspection address to assist and <br /> collect soil samples due to diesel release. This released was caused by a generator that <br /> Flee and smoke Clean-up caught fire. The fire department was contacted and assisted in controlling and putting out <br /> the fire. <br /> HAZMAT and <br /> Biohazard Clean-up On 5/24/21 ECOS Environmental and Disaster Restoration Inc. performed the initial <br /> Emergency Board-ups inspection and assessment of the spill site. At the time of the inspection, the generator <br /> and 2 control trailers were on site in the impacted area. A visual assessment was <br /> Animal Waste clean- performed in addition to a photoionization detector(P.I.D) which was used to help <br /> up evaluate and determine extent of spill. Soil samples were collected randomly from <br /> Storm Damage various areas as described in the diagram attached below. The collected samples were <br /> Repairs sent to an independent 3rd party laboratory for analysis. <br /> Carpet Cleaning On 5/26/21 ECOS Environmental and Disaster Restoration Inc. arrived on site as the area <br /> ...and more! was being excavated to remove impacted soil. The trailers and generator were removed <br /> so the area can be properly remediated. The impacted soil was being stored on site <br /> and placed on poly sheeting. The excavated area was randomly tested using the PID. The <br /> Of <br /> fice Locations soil was placed in bags to reach proper testing temperatures when collecting PID <br /> Aspen readings. Excavation was deemed complete as surface PID readings were below 1 part <br /> per million of Isobutene. Final confirmation samples were taken and sent to a third-party <br /> Boulder laboratory for analysis.
