Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> Y -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Lucas West of the Colorado Division of Reclamation,Mining and <br /> Safety as part of the normal monitoring program for permitted sites. Wade Wymore accompanied the inspection <br /> and represented the Operator. Normal activies were tkaing place at the mine during the inspection. at the King II <br /> area,the stackers and air jig were active and trucks were bineg loaded at the time of the inspection. At the Kind I <br /> facility refuse was being compacted and graded in preperation for more refuse to be added. The weather was clear <br /> and dry, seven photos accompany this report to illustrate the current site conditions. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> At the King II facility the bulk fuel and hazardous materials storage area was observed and can be seen in <br /> Photo One. All fuel,oils and lubricant containers being stored are in good condition with no evidence of leaks or <br /> spills. Various equipment items are being stored on the grates over top of the secondary containment structure to <br /> prevent any spillage of residual hydraulic fluids from escaping containment. The containment structure is in good <br /> condition and free from dirt and debris. <br /> Both the east and west clear water ditches at the King II facility were inspected. They appeared to be functioning <br /> as designed and were free from any trash or debris. Neither ditch shows any evidence of recent flows given the <br /> drought conditions of the area. An example of the east ditch can be seen in Photo Two, and the west in Photo <br /> Three. All internal water control ditches and culverts were functioning and unobstructed. The sediment pond <br /> located at King II had minimal standing water and can be seen in Photo Three. The pond was not discharging at <br /> the time of the inspection. <br /> At King I,all internal water control structures appeared to be functioning as designed with no evidence of recent <br /> flows. The visible ditches are clear and free from obstructions. The ditches report to the east and west sediment <br /> ponds respectively,which were also dry and in good condition. The in slopes of the pond are well vegetated and <br /> stable, and witness lines on the sediment markers indicate they have sufficient capacity.
