The following adequacy issues were noted:
• The E6-13 pad had been included on the map but not the table submitted with the SL-12
application. It had not been reclaimed at the time of the inspection and will not be included in
SL-12.An amended map was submitted by MCC.
• The E5-11 and E64 holes had been plugged and abandoned, but the pads had been left open as a
staging area. Since Phase I bond release requires that backfilling and grading of the disturbed
area be complete,E5-11 and E64 are not eligible for bond release and will not be included in
SL-12.
A follow-up inspection was made on May 7,2021,to verify the plugging and abandonment of the
monitoring wells(these were not marked on the map submitted with the SL-12 application and had been
overlooked during the first inspection).
The 17LC-01, -05, -06,and-07 exploration holes were also omitted from the June 3-5, 2019 inspection,
however their successful reclamation was documented in inspection reports from September and
October,2017, and in photographs sent to the Division.
The final list of sites to be released from Phase I liability with SL-12 are given below in Table 1.
Table 1
Phase Site Details Type Landowner Acreage Reclamation Comment
/manager Date
I e4-15 MVB USFS 0.54 2016
1 e4-16 MVB USFS 0.60 2016
1 e4-17 MVB USFS 0.63 2016
1 101we6x62 Exploration/MVB USFS N/A N/A Drilled on existing road
I e5-1 MVB USFS
I e5-2 MVB USFS 1.10 2017
1 e5-3 MVB USFS
I Access road to e5-1 through-3 Road USFS 0.07 2017
1 e5-4 MVB USFS 0.83 2017
1 e5-5 MVB USFS 0.46 2017
1 e5-6 MVB USFS
1 e5-7 MVB USFS 0.73 2017
I e5-8 MVB USFS 0.92 2018
1 e5-9 MVB USFS
1 e5-10 MVB USFS 0.85 2017
I 10we5x40 Exploration/MVB USFS N/A N/A Drilled on existing road
I 111we5x40 Exploration/MVB USFS
I e5-12 MVB USFS 0.75 2017
1 e5-13 MVB USFS
I e5-14 MVB USFS 0.59 2017
I Access road to e5-12 through- Road USFS 1.43 2018
14
1 e5-15 MVB USFS 1.84 2017
1 e5-16 MVB USFS 1.99 2017
1 e5-17 MVB USFS 0.32 2017
1 e5-18 MVB USFS 0.49 2017
C-1980-007 Page 5 of 10
