OUR4Y SILVER MINES <br /> 3.6.2 Monthly Inspection <br /> Monthly inspections are comprehensive and involve the following key elements: <br /> • Observe the exterior of all containers, pipes and other equipment for signs of <br /> deterioration such as leaks, corrosion and thinning; <br /> • Observe tank foundations and supports for signs of instability or settlement; <br /> • Observe pipes, valves and appurtenances for signs of poor connection or leakage; <br /> • Verify the proper functioning of fill level indicators and overfill prevention systems; <br /> • Observe berms for signs of deterioration or discharges of oil; <br /> • Check spill kit inventories; and <br /> • Check all secondary containment for damage and indication of container leakage. <br /> All concerns and indications of required maintenance are immediately reported to the <br /> Environmental Department. Pooled oil is removed immediately upon discovery. Repairs and <br /> other maintenance must be conducted as soon as possible. Written monthly inspection records <br /> are signed by the inspector and will be maintained in a central location for a period of 5 years. <br /> Monthly inspections are recorded and signed by the inspector using the Fulcrum App for data <br /> collection. <br /> 3.6.3 Annual Inspection <br /> OSMI personnel will conduct a comprehensive inspection annually that will replace a regular <br /> monthly inspection. The annual inspection is preferably conducted after a storm event.The <br /> Storm Water Management Plan also conducts inspections of the drainage control measures <br /> that complement the SPCC Plan and help prevent a discharge to Sneffels Creek. <br /> Written monthly inspection records are signed by the inspector and will be maintained in a <br /> central location for a period of 5 years. <br /> 3.6.4 Periodic Integrity Testing <br /> Any tank installed will receive an inspection to verify the tank is in good working order during <br /> initial filling. <br /> Spill Prevention Control and Countermeasure Plan 17 1 P a g e <br />
