COLORADO <br /> a Division of Reclamation, <br /> Mining and Safety <br /> Department of Natural Resources <br /> 12 May 2021 <br /> Ms.Miranda Kawcak <br /> Moffat County Mining,LLC. <br /> 29515 Routt County Road 927 <br /> Oak Creek,CO 80467 <br /> RE: Refuse Pile Report Review for 2019 Quarters 1,2,3,4. <br /> Williams Fork Mines,Permit No. C1981044 <br /> Dear Ms.Kawcak; <br /> The Colorado Division of Reclamation,Mining and Safety has completed its review of the above <br /> referenced reports. Please see the Division's findings below regarding the reports' compliance with the <br /> following Rules. <br /> 1. 4.10.2(1) <br /> a. The mine waste bank was inspected, on behalf of the person conducting surface coal <br /> mining operations, by a qualified registered engineer, or other qualified professional <br /> specialist. <br /> b. The reports are signed by you,the environmental manager. <br /> c. The 2019 Q1 report indicates the surface of the waste bank was covered with two feet <br /> of fill material in the fall of 2019. <br /> d. The 99 excavation waste disposal area was reclaimed in 2009. <br /> 2. 4.10.2(2) <br /> a. The submitted inspection reports complies with the inspection requirement of rule <br /> 4.09.1(11). <br /> o The reports reviewed comprised four quarterly reports. <br /> o The reports indicated the stability of the fills was inspected on all quarterly <br /> inspections and no appearances of instability, structural weakness or other <br /> hazardous conditions were noted. The placement and compaction of fill materials <br /> was also inspected each quarter. The inspections were conducted on: <br /> a. 26 March 2019 <br /> b. 18 June 2019 <br /> c. 26 September 2019 <br /> d. 11 November 2019 <br /> o The Division received the report on 29 March 2021. The Division requests that the <br /> operator submit the report so that the Division will receive it within the required <br /> two week time frame outlined in Rule 4.09.1(1 1)(c). <br /> b. The stability of the fills was inspected quarterly. <br /> o It does not appear tests were necessary to evaluate any potential hazard to human <br /> life and property.
