April 21,2021 C-1981-008/New Horizon Mine LJW <br /> permit area. The main area receiving backfill is the Morgan family property located on the northwest corner of <br /> 2700 and BB roads. Material is used to bring up low spots in conjunction with the landowner. This area has also <br /> been disced in preperation for planting at a later date. This area is shown in Photo Six. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage —Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> Pond 13 and it's discharge was observed during the inspection. The inflow to the pond was running <br /> through the aeration devices which appeared to be functioning as designed. The vegetation and banks of the pond <br /> were in good condition and stable at the time of the inspection. Photo Seven shows the aeration devices and the <br /> pond itself. The discharge point was also observed,no evidence of mineral staining was observed and the <br /> discharge was flowing unimpeded. Photo Eight shows the pond 13 discharge point. <br /> ENFORCEMENT ACTIONS/COMPLIANCE <br /> No enforcement actions were initiated as a result of this inspection, nor are any pending. <br /> Number of Partial Inspection this Fiscal Year: 7 <br /> Number of Complete Inspections this Fiscal Year: 3 <br /> Page 3 of 7 <br />
