Preliminary review of the information received determined the <br /> following items must be received before the Division can consider the application as being submitted and <br /> technical review can begin. Please respond to this Incompleteness Review with the requested <br /> additional/updated information on permit replacement pages and summarize each response in a cover <br /> letter titled "Incompleteness Review Response; M-2021-019". <br /> APPLICATION: <br /> 1. Question #10, Primary Mine Entrance Location,the coordinates given to not match the coordinates <br /> given for either of the mine entrances on the Existing Conditions or Proposed Contours maps. Please <br /> update the coordinates to be consistent with the maps. <br /> EXHIBIT B—Site Description (Rule 6.3.2): <br /> 2. The list of man-made structures within 200 feet of the affected area does not list the fiber optic line <br /> owned by AT&T Transmission as shown on the maps. Please update the list to reflect the fiber optic <br /> line. <br /> EXHIBIT D—Reclamation Plan (Rule 6.3.4): <br /> 3. Please indicate the rate that straw mulch will be applied. Based on the Division's experience with <br /> mulching rates two (2)tons of mulch per acre is recommended. <br /> 4. The applicant states in the mining plan that topsoil will be salvaged and stockpiled prior to mining <br /> and when mining is completed that topsoil will be spread 8-inches over the disturbed area.
