Mr. Frank Ferns of Elk Ridge Mining and Reclamation accompanied the inspection and represented the <br />Opearator. COVID-19 protocols were followed. The site is in final reclamation, and not active the day of the <br />inspection. The weather was clear and dry, as the region continues to experience drought conditions. <br />SUPPORT FACILITIES - Rule 4.04: <br />The site currently utilizes three hazardous materials storage areas. The first is a pair of used oil storage <br />tanks located on the south side of the shop Seen in Photo One. The tanks were mostly empty as verified by the fill <br />line indicator on the side of the tanks. The secondary containment structures were free from standing water or <br />excessive sediment. The containment liner appeared to be in good condition at the time of the inspection and no <br />problems were noted. <br />The second hazardous materials storage area is the bulk fuel tank located northeast of the shop area. This area is <br />the main fuel storage location for the site, where all equipment and mine vehicles are fueled and can be seen in <br />Photo Two. The storage tank was in good condition at the time of the inspection, The secondary containment <br />consists of berms lined with an HDPE liner, the liner did not contain any rips, tears or other signs of failure. The <br />containment area was free from standing water or excessive sedimentation. <br />The third hazardous materials storage area is located in the pole barns just west of the office building. This area is <br />used for storing various hydraulic fluids, oils and other small quantities of solvents and hazardous materials. All <br />materials stored in this area were stored in appropriate secondary containment devices that appeared to have <br />sufficient capacity to contain any potential spill. The area is covered, and unexposed to the natural elements as <br />shown in Photo Three. No problems were noted in this area during the inspection. <br />SIGNS AND MARKERS — Rule 4.02: <br />The appropriate Mine Identification sign was posted at the entrance to the site and all applicable boundary <br />markers were observed throughout the site. <br />Number of Partial Inspection this Fiscal Year: 3 <br />Number of Complete Inspections this Fiscal Year: 2 <br />Page 2 of 6 <br />
