a metal cap welded with well name identification scribed on cap. The final reclamation phase
was completed in the fall of 2010. The pad and access road was de-compacted and restored
to existing grade, stabilized topsoil was utilized to provide approximately 4" of topsoil covering
prior to seeding. The pad was drill seeded per the approved BLM seed mix, stabilized by weed
free straw mulch. The continued efforts of the noxious and annual weed management and spot
seeding created a diverse vegetation throughout the pad, including volunteer Rabbit and Sage
brush. CDRMS annual inspection determined the pad and the access road has good plant
establishment. There is spots throughout where the wildlife has grazed the vegetation to the
ground. The plant density is evident, the pad appears bare in spots due to the grazing but the
vegetation exists and will bounce back when pressure is decreased. Final reclamation has
been achieved.
65-27-199 (138-27)
The single installed Hydrology wells was plugged and abandoned Aug. 2003, the plugging
report submitted to CDRMS Sept. of 2003. The well casings were cut off at 4ft below final
grade, a metal cap welded with well name identification scribed on cap. The final reclamation
phase was completed in the winter of 2004. The pad and access road was de-compacted and
restored to existing grade, stabilized topsoil was utilized to provide approximately 4" of topsoil
covering prior to seeding. The pad was drill seeded per the approved BLM seed mix, stabilized
by weed free straw mulch. The continued efforts of the noxious and annual weed management
and spot seeding created a diverse vegetation throughout the pad, including volunteer Rabbit
and Sage brush. CDRMS annual inspection determined the pad had good vegetation diversity
with mature shrubs throughout the pad. The pad's has woody debris scattered and no signs of
erosion. Final reclamation has been achieved.
SIGNED STATEMENT:
All reclamation requirements of prospecting NOI P-2003-018 for the 2003 Hydrology program
— Public Land Prospecting Project have been satisfied by virtue of no new surface
disturbances being created at the proposed drill pad site or access route subsequent to NOI
issuance.
Signed: _ Printed Name: _Linda Weijers
8
