Construction Certification Report <br /> At the completion of the project, the CQA Monitor shall submit to MANAGER a Construction <br /> Certification Report. This report shall certify that the work has been performed in compliance <br /> with the DRAWINGS and SPECIFICATIONS and will contain the following information: <br /> ■ summary of all construction activities; <br /> ■ photographic documentation, <br /> • test data sheets; <br /> • copies of weekly reports; <br /> ■ CQA test results, including date, test locations and resolutions of deficient test results; <br /> • copies of surveyors certificate; <br /> ■ fill temperature monitoring results; <br /> ■ staff schedule summary; <br /> ■ a description of significant construction problems and the resolution of these problems; <br /> ■ changes to the DRAWINGS or SPECIFICATIONS and the justification for these changes; <br /> • record drawings, and <br /> ■ a statement certifying that construction was completed in compliance with the DRAWINGS <br /> and SPECIFICATIONS, signed, and sealed by the CERTIFYING ENGINEER registered in the <br /> State of Colorado. <br /> Page 9 <br />
