2 June 2020 C-1982-057/Seneca II -W Mine RAR <br />banks and functioning adequately. <br />SIGNS and MARKERS Rule 4.02 <br />o Entrance sign and permit boundary markers were posted as required. <br />o The mine site identification signs observed at the mine access points were intact, displaying <br />all required information: mine name, operator name, address, phone number, DBMS permit <br />number and contact information. <br />o The few topsoil piles remaining at the site were observed to be well marked and are generally <br />associated with roads. <br />ROADS Rule 4.03 <br />Construction 4.03.1(3), 4.03.2(3); Drainage 4.03.1(4), 4.03.2(4); Surfacing and Maintenance <br />4.03.1(5) and (6) 4.03.2(5) and (6). <br />o The haul road and access roads were in good repair, free of ruts and rills. <br />HYDROLOGIC BALANCE Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6,- Discharge <br />Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2,- Ground Water <br />Monitoring 4.05.13; Drainage- Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, <br />4.05.9; Stream Buffer Zones 4.05.18 <br />o Pond 5 was discharging at the time of inspection. Spillway and dam outslope appeared in <br />good repair. Weir was functioning adequately and in good repair. Some mesic grasses are <br />growing into the weir and the water is routing around them. <br />o Pond 6 slopes were well vegetated and stable. The pond was experiencing inflow and <br />was discharging at the time of inspection. The slump above appeared stable. <br />o Pond 9 was not discharging at the time of inspection. Discharge structure appeared in <br />good repair and dam outslope appeared stable and to be functioning adequately. Weir was <br />functioning adequately and in good repair. <br />o Pond 15 was discharging at the time of inspection. Discharge structure appeared in good <br />repair and dam outslope appeared stable and to be functioning adequately. Weir appeared to <br />be in good repair and functioning adequately. <br />o Pond 16 was receiving inflow from watershed above. Drop structure appeared to be <br />functioning adequately. Pond banks and spillway were in good repair at the time of <br />inspection. <br />o Pond 17 was discharging and appeared turbid at the time of inspection. Banks appeared <br />stable and functioning adequately. <br />o Constructed drainages appeared stable and to be functioning adequately at the time of <br />inspection. They appeared well armored. <br />o The wells above pond 6 were inspected and necessary repairs were noted. <br />TOPSOIL -Rule 4.06 <br />Removal4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br />o Topsoil piles, mostly associated with permanent roads and ponds, were observed to be <br />adequately marked, stable and vegetated. <br />Number of Partial Inspection this Fiscal Year: 8 <br />Number of Complete Inspections this Fiscal Year: 4 <br />
