NORTHCCV Emergency Response Plan <br /> NEWMONT <br /> (2) His or her immediate Supervisor who in turn will notify the Department Manager and the <br /> Functional Area Manager. <br /> (3) A representative from the Health and Safety Department who will in turn notify the Sr. <br /> HSLP functional area Manager who will advise the Regional Director of HSLP. <br /> (4) Confine the information to the facts of the accident. Radio discussions of the accident <br /> must be limited to emergency information only and names will not be broadcast. <br /> (5) Preserve the accident scene and all physical evidence. Accident site visits will be <br /> limited to only necessary emergency response personnel, security personnel and <br /> appropriate management. <br /> 5.17.6 The Investigation Team: <br /> (a) Due to the nature of fatalities, special emphasis is taken to investigate the accident, <br /> determine the causes and take the necessary steps to prevent any reoccurrence. All <br /> fatalities will be investigated by a Special Investigation Team, consisting of: <br /> (1) The Functional Area Manager <br /> (2) The Department Manager <br /> (3) The Superintendent of the Area Affected <br /> (4) Sr. HSLP Manager and Regional Director <br /> (5) Others, as determined by the Regional Director of Operations and as circumstances <br /> require, e.g. technical advisors. <br /> (6) The supervisor is responsible for completing a preliminary report of the accident. The <br /> Investigation Team will complete and distribute the final report as appropriate. <br /> 5.17.7 The Manager of External Relations will: <br /> (a) Be the sole and immediate contact with the Press. <br /> (b) Following the notification of next of kin, and with approval of the Executive Director of <br /> Operations, prepare and distribute a holding statement. <br /> (c) Notify the Head of Corporate P.R. <br /> 5.17.8 The Manager of Security will: <br /> (a) Secure the Company's security gates to prevent press officials and other unauthorized <br /> people from entering the mine property. <br /> (b) After receiving permission from the Director of HSLP, and in the presence of a witness, <br /> impound and catalog the personal effects of the deceased and forward them to the Director <br /> of HSLP who will provide to the Director of Human Resources for delivery to the deceased's <br /> family. <br /> (c) Notify the appropriate Coroner and Police departments of the fatality. <br /> 5.17.9 Regional Director HSLP: <br /> (a) Notify the Newmont Global HSLP Director <br /> (b) Give approval to notify external agencies <br /> (c) Brief the Operations Director on the facts of the accident <br /> (d) Notify the Director of Security <br /> (e) Promptly notify the facts of the fatality or injury to the Director of Human Resources <br /> (f) Promptly notify Regional Legal Counsel. <br /> 5.17.10 Regional Legal Counsel will: <br /> (a) Contact corporate legal to discuss the work related fatality. <br /> (b) Contact the retained Legal MSHA consultant to advise and discuss. <br /> Department THIS DOCUMENT IS UNCONTROLLED IN HARD COPY FORMAT Doc Id:NA-CCV-HSLP-PLAN-1775 <br /> Health Safety&Loss Prevention Date of This Issue:25 June 2018 Date of Next Review:25 June 2019 Page 29 of 38 <br />
