NEWMONT <br /> GOLDCORR <br /> monitor observed that the DCF depth was insufficient, the contractor was notified <br /> and remediated the issue. <br /> The DCF isopach map (Drawing No. 11) was developed by generating a volume <br /> surface between two surfaces, the top of the SLF and the top of the DCF. The SLF <br /> and DCF surfaces were created from survey points. All survey points were not <br /> taken in the same location for each surface. The depths showing less than 2 feet <br /> have some level of error associated with them due to the surface triangulation <br /> between points. Additionally, after the DCF isopach map was created, NewFields <br /> identified and verified DCF depths in areas of concern by hand-potholing and <br /> measuring the depth in the field. <br /> Appendices: <br /> 10. Appendix E: Both of the Soil Liner Acceptance Forms for August 20, 2019 and August 22, <br /> 2019 include Panel P-3095 while all other acceptance forms were sequential and did not <br /> have an overlap. Please clarify if the SLF acceptance for this panel area took more than <br /> one shift and how the SLF conditioning was maintained overnight. <br /> After a review of the Soil Liner Acceptance Forms and cross referencing the dates <br /> and panel numbers with the Appendix JA — Deployment Summary it was <br /> determined that the inclusion of P-3095 on August 22, 2019 was a clerical error and <br /> the beginning panel should have been P-3096. The SLF was covered by P-3095 on <br /> August 20, 2019 and was not exposed on August 22, 2019. <br /> 11. Appendix E: The Soil Liner Acceptance Forms for September 9, 2019 through September <br /> 12, 2019 include a comment regarding the subgrade being exposed and recovered within <br /> 15 minutes. Please provide some narrative addressing: <br /> a. Why this was necessary? <br /> As discussed in Section 2.10 Miscellaneous section of the report, the <br /> geomembrane was removed and replaced within the footprint where a mine <br /> haul truck drove into the project. The work associated with the <br /> geomembrane removal and replacement, including the removal and <br /> replacement of DCF,was completed between September 3rd and 13th,and the <br /> removal and replacement of the geomembrane occurred between September <br /> 9th and 121. This is when the SLF was exposed and the density and moisture <br /> were re-tested. <br /> b. Elaborate as to whether this was before or after the SLF was placed and/or <br /> accepted. <br /> C. <br /> This was done after the initial approval. <br /> d. What is the significance of the 15-minute apparent time limit? <br />
