NEWMONT
GOLDCORR
monitor observed that the DCF depth was insufficient, the contractor was notified
and remediated the issue.
The DCF isopach map (Drawing No. 11) was developed by generating a volume
surface between two surfaces, the top of the SLF and the top of the DCF. The SLF
and DCF surfaces were created from survey points. All survey points were not
taken in the same location for each surface. The depths showing less than 2 feet
have some level of error associated with them due to the surface triangulation
between points. Additionally, after the DCF isopach map was created, NewFields
identified and verified DCF depths in areas of concern by hand-potholing and
measuring the depth in the field.
Appendices:
10. Appendix E: Both of the Soil Liner Acceptance Forms for August 20, 2019 and August 22,
2019 include Panel P-3095 while all other acceptance forms were sequential and did not
have an overlap. Please clarify if the SLF acceptance for this panel area took more than
one shift and how the SLF conditioning was maintained overnight.
After a review of the Soil Liner Acceptance Forms and cross referencing the dates
and panel numbers with the Appendix JA — Deployment Summary it was
determined that the inclusion of P-3095 on August 22, 2019 was a clerical error and
the beginning panel should have been P-3096. The SLF was covered by P-3095 on
August 20, 2019 and was not exposed on August 22, 2019.
11. Appendix E: The Soil Liner Acceptance Forms for September 9, 2019 through September
12, 2019 include a comment regarding the subgrade being exposed and recovered within
15 minutes. Please provide some narrative addressing:
a. Why this was necessary?
As discussed in Section 2.10 Miscellaneous section of the report, the
geomembrane was removed and replaced within the footprint where a mine
haul truck drove into the project. The work associated with the
geomembrane removal and replacement, including the removal and
replacement of DCF,was completed between September 3rd and 13th,and the
removal and replacement of the geomembrane occurred between September
9th and 121. This is when the SLF was exposed and the density and moisture
were re-tested.
b. Elaborate as to whether this was before or after the SLF was placed and/or
accepted.
C.
This was done after the initial approval.
d. What is the significance of the 15-minute apparent time limit?
