October 14, 2019 C-1994-082/toast Mine RAR <br /> HYDROLOGIC BALANCE-Rule 4.05 <br /> General Requirements 4.05.1; Effluent Standards 4.05.2; Divergence and Conveyance of Overland <br /> Flow 4.05.3; Diversions 4.05.4; Sediment Control Measures 4.05.5,4.05.6; Discharge Structures 4.05.7 <br /> Impoundments 4.05.9; Ground Water Monitoring 4.05.13; Underground Mine Entry 4.05.10; <br /> Surface and Ground Water Monitoring 4.05.13;Stream Buffer Zones 4.05.18: <br /> Unless otherwise noted pond banks, dams and reconstructed drainage ways appeared stable, functioning <br /> adequately and well vegetated at the time of inspection. <br /> o Pond 10,was not discharging at the time of inspection. The outslope of the dam was stable with no seeps <br /> or evidence of leakage observed at the time of inspection. <br /> o The following drainages: <br /> IPM3, IPM3,YPM6 and YPM7 were walked and appeared well armored and stable withwell established <br /> vegetation on banks and grasses taking hold in some channels. No issues were noted at the time of inspection. <br /> SLIDES and OTHER DAMAGE-Rule 4.12 <br /> o Existing slides were noted as stable at time of inspection with no recent movement or new instability <br /> observed. <br /> o Due to slope and aspect, slide and slumped areas tend to be sparsely vegetated. <br /> TOPSOIL -Rule 4.06 <br /> Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br /> o Topsoil piles observed were associated with roads. Their slopes appeared stable and vegetated. Some <br /> Bromus tectorum(cheat grass),was observed on topsoil piles,particularly on south facing slopes. <br /> REVEGETATION-Rule 4.15 General Requirements 4.15.1; <br /> o Reclamation and revegetation have been completed at this site. <br /> o No extensive areas devoid of vegetation or, expansive areas of weeds were noted at the time of inspection <br /> with the exception of small patches of Bromus tectorum(cheat grass), on hot, south facing slopes. <br /> o Reclamation appeared stable with well established vegetation. <br /> o Vegetation was going dormant at this time of year. <br /> o A diversity of species comprising forbes, grasses and shrubs was observed throughout the <br /> site over the course of this inspection. <br /> o Establishing vegetation on slide and slump areas is problematic due to slope and aspect. <br /> SLIDES and OTHER DAMAGE-Rule 4.12 <br /> o The site, in the past, has experienced soil movement in the form of slumps and slides. At <br /> The time of inspection no evidence of recent new movement was observed and historic slide <br /> areas appeared stable. <br /> INSPECTION, ENFORCEMENT and CIVIL PENALTIES—Rule 5 <br /> Inspections 5.02; Availability of Records <br /> Records were inspected and the following deficiencies were noted: <br /> o Quarter 3 2019 Sediment Pond Inspections were in progress but not yet available in the public file at the <br /> mine site. <br /> Number of Partial Inspection this Fiscal Year: 2 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 3 of 9 <br />
