PERMIT#:M-2011-040 <br /> INSPECTOR'S INITIALS:DMC <br /> INSPECTION DATE:August 13,2019 <br /> 3.1.12(1) of the rule. <br /> CORRECTIVE ACTIONS:The operator shall, at the entrance of the mine site, post a sign which shall be <br /> clearly visible from the access road with the following:the name of the operator, a statement that a <br /> reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation Board; and <br /> the permit number. The operator shall submit photo documentation that a proper sign has been posted <br /> by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 10/15/19 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the Division of Reclamation, Mining and Safety's (Division) normal <br /> monitoring program. A representative for the operator was not present. Photographs are included with this report <br /> in order to illustrate some of the conditions observed. <br /> The Prince Albert Mine is located approximately 2.5 miles west of Uravan and accessed from U16 Road. The site <br /> is a 110d operation that includes a total of 8.3 permitted acres. The Division currently holds a financial warranty <br /> amount of $15,087.56 for this site. The Division has reviewed the reclamation costs, in order to ensure the <br /> financial warranty reflects the actual current cost of fulfilling the requirements of the Reclamation Plan, and <br /> determined that the current financial warranty amount is sufficient. <br /> Pursuant to Rule 1.13.5,any operator who temporarily ceases production of the mining operation for one hundred <br /> eighty (180) days or more must file a Notice of Temporary Cessation with the Division. The initial period of <br /> Temporary Cessation may be up to five years. The site was placed into Temporary Cessation status on October <br /> 7,2013. On November 11,2018 the operator notified the Division that the site was returning to active status. This <br /> inspection found that there has been no mining activity at the site. Therefore, the operator must request a second <br /> five year period of Temporary Cessation pursuant to Rule 1.13.5 (3). Alternatively, the operator may commence <br /> final reclamation of the mine site. This is noted as a problem in this report. <br /> Adequate mine identification signage was not noted at the entrance to the site. This is noted as a problem in this <br /> report. <br /> The affected area was marked by t-posts that were adequate to delineate the affected area boundary. <br /> There is no ore stored on the ore pad. Stormwater controls appear effective. No sedimentation or erosion issues <br /> were observed. The catchment pond was dry during this inspection.Noxious weed infestations were not noted. <br /> No other problems or violations were noted during this inspection.
