OPERATORS <br /> SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br /> WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br /> Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br /> Reclamation Board will consider possible enforcement action. <br /> 1. INSPECTION TOPIC:Availability Of Records <br /> PROBLEM/POSSIBLE VIOLATION: Problem: Rule 1.13.5 states, if an Operator temporarily ceases <br /> production of the mining operation for one hundred eighty (180) days or more,the Operator must file a <br /> Notice of Temporary Cessation in writing to the Office. The site was placed into Temporary Cessation <br /> status on October 7, 2013. On November 11, 2018 the operator notified the Division that the site was <br /> returning to active status. However,this inspection found that there has been no mining activity at the <br /> site. <br /> CORRECTIVE ACTIONS:The Operator must request a 2nd period of Temporary Cessation, to be effective <br /> October 7, 2018, or commence final reclamation. <br /> CORRECTIVE ACTION DUE DATE: 10/15/19 <br /> 2. INSPECTION TOPIC:Signs& Markers <br /> PROBLEM/POSSIBLE VIOLATION: Problem:The mine identification sign was not posted at the entrance of <br /> the mine site.This is a problem for failure to post a mine identification sign as required by Section <br /> Page I of 4 <br />
