August 26, 2019 C-1981-044/Williams Fork Mines RAR <br /> INSPECTION.- <br /> GENERAL MINE PLAN COMPLIANCE: <br /> Reclamation is underway as permitted, with all of the facilities demolished in the area along the Williams <br /> Fork River. Remaining facilities include a 60 in culvert at the portal area and an electrical transfer <br /> station. Work has been ongoing at the shop area and currently consists of grading in preparation of <br /> topsoil laydown. A grader was operating at the time of inspection. Portal area backfilling is underway. <br /> Trucks were transporting backfill material to the portal at the time of inspection. The site appeared neat <br /> and orderly at the time of inspection <br /> SIGNS AND MARKERS Rule 4.02 <br /> o Topsoil stockpiles were marked as per the rule. <br /> o Mine site identification signs to the site from public road, CO Road 13, were intact, <br /> displaying all required information: mine name, operator name, address, phone number, <br /> DRMS permit number and contact information. <br /> ROADS-Rule 4.03 <br /> Construction 4.03.1(3)14.03.2(3);Drainage 4.03.1(4)14.03.2(4); Surfacing and Maintenance 4.03.1(5) and(6)I <br /> 4.03.2(5) and(6);Reclamation 4.03.1(7)14.03.2(7): <br /> o Main haul roads in and around the site appeared stable, adequately maintained and free of <br /> ruts. <br /> o Some fugitive dust was noted due to heavy equipment travelling on the dry roads. However, <br /> no off site air quality impacts were observed at the time of the inspection. <br /> HYDROLOGIC BALANCE—Rule 4.05 <br /> Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, <br /> 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Drainage-Acid <br /> and Toxic Materials 4.05.8; Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18 <br /> Ponds observed appeared dry. Pond banks appeared stable at the time of inspection. The spring was not <br /> discharging at the time of inspection. (photo) <br /> Alluvial Valley Floor designation along the Williams Fork River was checked and appears reasonable. <br /> (get the reclamation guidelines from the rules0 <br /> TOPSOIL-Rule 4.06 oval4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3,Redistribution <br /> 4.06.4: <br /> o Many small topsoil piles exist throughout the site, and will be used in the reclamation activities to <br /> achieve approximate original contour. <br /> o Slopes appeared stable. <br /> BA CKFILL and GRADING—Rule 4.14 <br /> o Backfilling and grading operations appeared to be progressing as permitted. <br /> o Work had recommenced at the portal. <br /> o Grading operations were taking place in the facilities area along the Williams Fork River. <br /> o Topsoil is stockpiled in several places in this area ready for laydown. <br /> Number of Partial Inspection this Fiscal Year: 1 <br /> Number of Complete Inspections this Fiscal Year: 1 <br /> Page 3 of 9 <br />
