Squaw Gulch — 19VLF Phase 2A-1 and 2A-3 High Volume Collection Pipe Asbuilts, <br /> Drawing 2 of 3: The drawing does not include a professional Land Surveyor (PLS) or <br /> Professional Engineer (PE) stamp, signature or date. Pursuant to Rule Rules 6.2.1(2) <br /> and 7.3, drawings require a date, and registered land surveyor/professional engineer <br /> signature. Please resubmit this drawing with the required professional signature, date <br /> and stamp. <br /> This drawing has been updated to meet requirements of Rule 6.2.1(2) and 7.3, and <br /> is enclosed in this response. <br /> 4. Squaw Gulch — 19VLF Phase 2A, Part 1 Drain Cover Fill As-Built /sopach: This drawing <br /> represents 14 smaller areas where the drain cover fill is less than the required 2 feet. <br /> Please provide some discussion as to whether this was field checked and how these <br /> areas were brought up to specification. <br /> DCF is placed within the project limits with a low ground pressure D6 dozer with <br /> GPS capability. NewFields QA/QC representatives constantly monitored placement <br /> of DCF. Direct measurement of the DCF with a tape measure was performed if the <br /> DCF depth appears to be less than 2 feet based on visual inspection. If the CQA <br /> monitor observed that the DCF depth was insufficient, the contractor was notified <br /> and remediated the issue. <br /> The DCF isopach map (ROC Drawing 6)was developed by generating a volume <br /> surface between two surfaces, the top of SLF and top of DCF. The SLF and DCF <br /> surfaces were created from survey points. All survey points were not taken in the <br /> same location for each surface. The depths showing less than 2 feet have some <br /> level of error associated with them due to the surface triangulation between points. <br /> Additionally, after the DCF isopach map was created, NewFields identified and <br /> verified DCF depths in the field in areas of concern. <br /> 5. Section 3.4.2 — 40 Mil Smooth Liner: Roll #F17C003376 is missing a date and QC <br /> signature similar to the other QC certifications for 80 mil. The date of the 40-mil liner is <br /> as old as 2014. The DRMS could not find any statement in the report indicating how the <br /> liner that was delivered to the site and not used until this project was stored. Please <br /> address this discrepancy. <br /> The signed and dated QC certification for Roll #F17C003376 (40mil geomembrane) <br /> is included as Attachment 1. <br /> The roll of 40mil geomembrane delivered in 2014 was stored elevated off the <br /> ground, on top of pipes oriented parallel with the roll.
