Homestake Mining Company May 10,2019
Regular(112d)Operation Reclamation Permit Application Package
• The Sericite Stockpile will be removed and placed as cover material, and the footprint of
the former stockpile will be scarified and graded to prepare it for seeding.
• The Low Point Mitigation located at approximately elevation 10,620 ft. will be graded to
drain and avoid ponding water.
• Piezometers will be maintained for water level measurement and water quality sampling
as long as the Water Management OU is in operation. Additional piezometers may be
installed or replaced as necessary as the groundwater monitoring program evolves.
Following reclamation of this Water Management OU, piezometers will be plugged and
abandoned using standard procedures or retained for future operating use as needed.
Well stickups will be removed and surrounding well pads will be reclaimed and
revegetated. All materials removed during wellhead abandonment will be properly
disposed of. After surface features of the abandoned wells have been removed, the site
will be graded to prepare it for seeding.
• Extraction and injection wells, water conveyance piping, and equipment used for water
recirculation and reagent injection as part of the water quality management systems will
be maintained as needed for insitu treatment; for the full duration over which the Water
Management OU is in operation. Additional injection/extraction wells may be installed or
replaced as necessary during that time, with approval obtained through Technical
Revision requests.
• On or before reclamation of the Water Management OU, injection/extraction wells will be
plugged and abandoned using standard procedures or retained for future operating use.
Well stickups will be removed and surrounding well pads will be reclaimed and
revegetated. All materials removed during wellhead abandonment will be properly
disposed of. After surface features of the abandoned wells have been removed, the site
will be graded to prepare it for seeding.
Existing roads and drainage features within this operable unit are located on private property and
will remain open to accommodate general site access and surface water management.
The IRD will be regraded to the final topography shown on Exhibit F Map F-0313, with a section
presented on Exhibit F Map F-03C.
5.4. Operable Unit 4: Tie Camp Rock Dump
The current topography for the TCRD is shown on Exhibit F Map F-04A.
5.4.1. Previous Reclamation
The majority of the TCRD, including existing roads, drainage features, and the low-grade ore
stockpile, has been regraded to its final slopes, terraced, and reseeded. A 36" Corrugated Metal
Pipe (CMP) has been installed to convey collected stormwater from the top of the TCRD to the
bottom to mitigate erosion on the face of the dump slopes. Per the Minerals Program Inspection
Report performed on July 25, 2018 (CDRMS, 2018b), the slope below the access road still
appears stable and vegetation is well established on the slope.
EXHIBIT E-RULE 6.4.5(AMENDED RECLAMATION PLAN) Page 70
HOMESTAKE MINING COMPANY
