Maintenance work may be done anytime within daylight hours as long as no interference to traffic is <br /> caused. <br /> 43. MATERIALS <br /> 44. Disturbed right-of-way area, where applicable, must be top soiled, fertilized, mulched, and reseeded <br /> with Department of Transportation Specification grass seed mix, which meets specifications <br /> 45. Please refer to attached sheet for CDOT seeding specifications for site revegetation. <br /> 46. MISCELLANEOUS <br /> 47. All construction vehicles to have mounted flashers. <br /> 48. The utility shall avoid disturbing or damaging all highway property, and shall be responsible for the <br /> prompt reconstruction, alteration, repair, or maintenance of highway property, as necessary, to repair any <br /> damage caused by the accommodation of the utility, and to restore the highway to pre-existing or other <br /> conditions as may be specified in a permit or agreement authorizing the accommodation. <br /> 49. Any retired facilites shall remain the utilities sole responsibility, and is subject to all provisions of the <br /> code and all terms and conditions of the permit issued for that faciliity. Maintain records of such facilities <br /> and respond to locate notices and requests from the UNCC or others. In providing such services the utility <br /> shall indicate the facility is abandoned. <br /> 50. CDOT Staff Bridge will conduct inspection of this structure to meet CDOT asset management goals <br /> and manage the risk to CDOT. Miscellaneous structures are inspected in accordance with National Bridge <br /> Inspection Standards defined in Federal Regulation 23 CFR 650 Subpart C except for inspection <br /> frequency. CDOT has defined the default routine inspection frequency for miscellaneous structure as 48- <br /> months. <br /> 26. CDOT RESERVES THE RIGHT TO REQUIRE THE PERMIT FEE TO REMOVE ALL PORTIONS <br /> OF THIS CONVEYOR BELT FROM CDOT RIGHT-OF-WAY FOR ANY REASON. UPON RECEIPT <br /> OF A WRITTEN REQUEST FROM CDOT, THE PERMITTEE SHALL ENACT THE AGREED-UPON <br /> REMOVAL PLAN AS SOON AS FEASIBLE. AT A MINIMUM, A TIMELINE OF THE REMOVAL <br /> SHALL BE PROVIDED TO THE DEPARTMENT WITHIN 45 DAYS OF WRITTEN NOTICE TO <br /> REMOVE. ALL COSTS OF THE REMOVAL, INCLUDING ALL DESIGN COSTS, SHALL BE BORN <br /> ENTIRELY BY THE PERMITTEE. <br />
