State Hichwav Utility Permit 4 <br /> Attachment to Permit No. 4190361-Additional Terms and Conditions Version 20 <br /> 27. No storage of materials or equipment will be allowed within CDOT right-of-way or 30 feet off the edge <br /> of the raveled way, whichever is greater. <br /> 28. TRAFFIC CONTROL <br /> 29. No work will be allowed at night, or on Saturdays, Sundays, and legal holidays without prior <br /> authorization from the Department. The Department may also restrict work within the State Highway <br /> right-of-way during adverse weather conditions. <br /> 30. Traffic control will be as per the approved submitted MHTS. <br /> 31. Traffic control plans are to be reviewed and accepted before construction. <br /> 32. The Permittee shall coordinate all traffic control operations with any special events in the area. The <br /> Permittee shall not schedule operations that interfere with traffic from 2 hours before until 2 hours after any <br /> such event. <br /> 33. Two-way traffic must be maintained through the area at all times unless shown otherwise in approved <br /> MHT. <br /> 34. No equipment will be allowed in the main lanes of the roadway during construction unless pre- <br /> approved by the Department in a valid traffic control plan. <br /> 35. Construction signs when not used shall be removed from the clear zone. All work that requires traffic <br /> control shall be supervised and implemented by a Traffic Control Supervisor certified by the American <br /> Traffic Safety Services Association (ATSSA) or the Colorado Contractors Association (CCA). When flag <br /> persons are required they shall be properly certified. <br /> 36. A certified Traffic Control Supervisor shall be on site at all times while construction traffic control is <br /> being utilized on the project. <br /> 37. New above ground installations shall be placed outside of roadway "clear zone" as defined by the latest <br /> AASHTO Roadside Design Guide or the installation shall have a breakaway system reviewed and <br /> approved by CDOT. <br /> 38. Lane closures or work within clear zone shall not be allowed during inclement weather and/or when the <br /> roads are wet, snow covered or icy. If you have any questions regarding whether it is safe to set up traffic <br /> control or work, contact your CDOT inspector. <br /> 39. SURFACING <br /> 40. PER SECTION 304.04 of the CDOT Utility Manual: "Utility shall replace any removed pavement at a <br /> design equal to or greater than the surrounding, undisturbed pavement structure". <br /> 41. MAINTENANCE OF UTILITY <br />
