(1) Material extraction will begin on the Northeast end of the pit and progress . <br /> westerly, turning south at the western most boundary. Material extraction will <br /> progress in a zig-zag pattern working from north to south, east to west and west to east. <br /> (2) Approved map shows a 200-foot buffer along the access road to the historic well with a <br /> 200-foot buffer around the well. <br /> (3) Access to the borrow pit shown in the Northwest corner of the affected area. <br /> Revised Mining Plan: <br /> (1) Material extraction will begin on the Southeast end of the pit and progress westerly, turning <br /> north at the western most boundary. Material extraction will progress in a zig-zag pattern <br /> working from south to north, east to west and west to east. Please see revised Mining Map as <br /> mining progression exhibit. The revised mining progression has been included in the <br /> Reclamation Map for information only. <br /> (2) Revised Map shows a 200-foot buffer around the perimeter of the well with the access road <br /> of the well being mined. Westerly extent of the affected area has been moved to the east to <br /> account for the additional disturbance from mining through the road to maintain the affected <br /> area under 30 Acres. <br /> (3) Access to the borrow pit has been updated to reflect actual access installed to allow better <br /> site distance for trucks exiting the site. Access road is approximately midway between the east <br /> and west boundaries at the north side of the site. <br /> Problem 2: The affected area boundary markers are missing or incorrectly placed. This is a <br /> problem for failure to maintain boundary markers around the affected area as required by Rule <br /> 3.1.12(2). <br /> Corrective Action 2: <br /> The Operator shall conduct a survey and replace the boundary markers in the correct <br /> location(s). The Operator shall provide proof to the Division that this has been done by the <br /> corrective action date. <br /> Response: <br /> The required survey has been completed and the corner boundary markers have been placed in <br /> the correct location(s) as requested. Photographic documentation of the boundary markers is <br /> attached at the end of this package. <br />
