Mr. Peter Mays, DRMS <br /> May 17,2019 <br /> (e) The excavation necessary to investigate the cause commenced on May 15. <br /> Approximately 8 feet below the surface a 2" HDPE pipeline connected to MLEX-5 was <br /> leaking at the pitless adapter due to a misalignment between the pipe and the well. Most <br /> of the flow was kept in the pipeline but a small amount was leaking from this location.
