MLEX-5 intercepts <br /> process water that could potentially impact groundwater and returns it to the tailing storage facility. On the <br /> morning of May 10, during routine inspections, Henderson personnel noticed that water was pooling on the <br /> surface surrounding the MLEX-5 well head at a very slow rate, estimated to be no more than two to three <br /> gallons per minute. Power to the well pump was disconnected and very shortly afterwards the water <br /> stopped flowing and began to subside indicating that there was a subsurface leak related to the well. At <br /> this time the Environmental Department was notified. <br /> The following paragraphs correspond to the elements of the follow-up written notice set forth in Rule 8.2.3, <br /> paragraphs (a) through (e), of the DRMS Rules and Regulations: <br /> (a) After discovering the issue, Henderson employees disconnected power from the pump and <br /> locked out the controls to stop any additional release. <br /> (b) The water on the surface did not adversely impact human health, property or the <br /> environment. This is based on the known chemistry of the water and the location of the <br /> event.
