April 9, 2019 C-1981-018/Deserado Mine CCW <br /> <br /> <br />Number of Partial Inspection this Fiscal Year: 6 <br />Number of Complete Inspections this Fiscal Year: 4 <br /> <br /> Page 10 of 13 <br /> <br />- RDH-4 <br />- Water Storage Tank and Radio Tower <br />- West Mains Vent Shaft <br />- “Treasure Island” Haul Road Blister <br /> <br />Support Facilities that were inspected, were in good condition. Pads fo r the facilities were well maintained and <br />stable. There were no major concerns with erosion. The access road to the north side of the Nitrogen #2 Plant was <br />repaired since the last inspection. The culvert at the same location was noted to be plugged during the March 2019 <br />inspection. The culvert was located and unplugged during this inspection (Photo 16). Rills coming off the fill <br />slope of the 13L Return Shaft pad were identified. The rills were not significant enough to create concerns of <br />instability at the site. The Division recommends filling and regrading thee rills to prevent further erosion. <br /> <br />Photo 16: The outlet of the culvert located at the Nitrogen #2 Plant. <br /> <br />SIGNS AND MARKERS – Rule 4.02: <br /> The mine identification sign was located to the right of the entrance to the permit boundary on County <br />Road 65. The sign displayed the required information about the permit number, the Permittee and the Division. <br />Topsoil markers throughout the site were easily identified and placed in the correct locations. Due to the size of <br />the topsoil pile above the main facilities area, the Division recommends placing another topsoil marker on the <br />southern portion of the pile (at the top of the pile). <br /> <br />TOPSOIL – Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Pro tection 4.06.3; Redistribution 4.06.4: <br /> Topsoil piles located throughout the mine site were in good condition. The piles were stable with well- <br />established vegetative cover on them. There were no indications of erosion or soil loss. The perimeter ditches <br />were in good repair and functioning as designed. <br /> <br />
