Mr. Peter Hays <br /> RE:Lazy Two Pit M-2019-001 <br /> April 17, 2019 <br /> 8. The applicant commits to providing the Division with a technical revision prior to the <br /> operation of a temporary asphalt plant on the Lazy Two Pit site. <br /> 9. The fueling/lubrication area near the office/scale house will be a dedicated location. Minimal <br /> amounts of bulk fuel and miscellaneous equipment maintenance fluids and oils will be stored <br /> on-site. When possible, fuels, oils, and other hazardous materials will be brought to the site <br /> as-needed with mobile maintenance vehicles. If stored on-site, bulk hazardous materials will <br /> be stored in double-walled tanks within lined and bermed secondary containment. The <br /> vehicle pads shall be prepared with compacted clay and safety equipment and materials to <br /> immediately clean up, store, and dispose of any spilled hazardous materials will be kept <br /> on-site. <br /> 10. The statement in Exhibit E, Reclamation Plan, on Page 10 "All available topsoil material will <br /> be used for backfilling." Has been changed to "All available overburden material will be <br /> used for backfilling." <br /> 11. The statement in Exhibit D, Mining Plan, that topsoil shall be removed and segregated from <br /> overburden is correct. As topsoil and overburden are stripped prior to mining, the materials <br /> shall be segregated and stockpiled separately for use during reclamation. The statement in <br /> Exhibit E, Reclamation Plan, on Page 11 more accurately describes all stripped material <br /> available for reclamation. Therefore, the statement has been changed from, "approximately <br /> one million cubic yards (cyds) of combined topsoil and overburden will be available for <br /> backfilling" to, "approximately one million cubic yards (cyds) of combined topsoil and <br /> overburden will be available for reclamation". The revised Exhibit E, Reclamation Plan, is <br /> attached. <br /> 6.4.7 Exhibit G—Water Information <br /> 12. The owners of properties that could potentially have wells within 600 feet of the affected <br /> land were contacted directly, including site visits, to evaluate the presence of Colorado <br /> Division of Water Resources registered and/or unregistered groundwater wells. Property <br /> owners that were contacted included Ross, Burris, Mellott, and Maclean. Wells that were <br /> located are listed and illustrated on the attached revised Exhibit C-4, Water Resource Map. <br /> In Exhibit G, Water Information, the statement, "The only registered groundwater well <br /> within 600 feet of the proposed permit area is owned by the proposed permit property owner <br /> (see Exhibit C-4, Water Resource Map)" has been changed to, "Known Colorado Division of <br /> Water Resources registered and/or unregistered groundwater wells within 600 feet of the <br /> affected area are illustrated on Exhibit C-4, Water Resource Map." A revised Exhibit G, <br /> Water Information, is attached. <br /> 13. The source of water used on the Lazy Two Pit site will be the groundwater well located at the <br /> Tucker Aggregate Pit, M-1980-002. The approved Colorado Division of Water Resources <br /> well permit is attached. <br /> 14. Groundwater on the Lazy Two Pit site will not be exposed during mining or reclamation. If <br /> groundwater is encountered, the area will be backfilled to two (2) feet above the normal <br /> blue earth Page 3of6 <br />
