March 20, 2019 C-1981-018/Deserado Mine CCW

Number of Partial Inspection this Fiscal Year: 6
Number of Complete Inspections this Fiscal Year: 3

- Halandras Landfill
- Water Storage Tank
- Radio Tower
- Conveyor Corridor
- West Main Vent Shaft
- B Vent Shaft No. 1

The pads for the support facilities were well maintained. Berms at the sites were intact and in good repair. Minor
rutting was observed at a couple of locations. The culvert on the north side of the Nitrogen Plant #2 was plugged
at the outlet. Noted in the February 2019 Inspection Report, ice had plugged the inlet to the culvert and runoff was
washing over the access road. The inlet to this culvert was unplugged during this visit. However, the outlet of the
culvert had become buried in sediment and gravel that had washed off the road. Pease locate and clear out the
outlet of this culvert to restore it. The area of the access road where water had previously washed over it
should be repaired in the spring as a part of the sprin g maintenance.

SIGNS AND MARKERS – Rule 4.02:
The mine Identification signs are displayed at the entrance to the mine site. The signs displayed the
required information about the Permittee, the permit number and the Division. The signs were in good condition.

DOCUMENTS RECEIVED: None
OTHER (SPECIFY): None
