Newmont Mining Corporation <br /> Record of Construction Report ■ <br /> VLF2-Removal of the Crushed Ore Stockpile <br /> &Liner Verification Project <br /> NewFields Project No.47S.0106.030 <br /> December 17,2018 <br /> covering the metal cutting edge,Ames carefully removed the DCF material to approximately 4"above the <br /> liner. Using hand shovels,Ames removed the remaining material to expose the liner. Each pot hole was <br /> approximately 2' in diameter. Additionally, during the excavation of the pot holes,the contractor noted <br /> a hard crust on the surface of the DCF. From our previous experience pot holing on VLF2, if the DCF was <br /> difficult to excavate into,it indicated that the DCF had not been displaced and there was not any damage <br /> to the geomembrane below. However, if it was easy to excavate into the DCF,this indicated that the DCF <br /> had moved and possibly damaged the geomembrane below. The location of the pot holes is shown on <br /> Figure 1. <br /> The geomembrane liner at each pot hole was examined and photographed by a qualified NewFields <br /> representative and Newmont representatives. Based on inspection of the geomembrane during the pot <br /> holing activities,only cosmetic scratches from exposing the liner were present and no significant damage <br /> or holes were observed. See Appendix A for the pot hole photo log. <br /> 3.0 DAILY OBSERVATION REPORTS <br /> Field daily observation reports prepared by NewFields are presented in Appendix B. <br /> 4.0 ENGINEERS OPTION <br /> After reviewing the all the evidence, it is the opinion of NewFields that the 80 mil LLDPE geomembrane <br /> was not damaged during the development or removal of the crushed ore stockpile and the integrity of <br /> the liner has not been compromised. <br /> If you have any questions or require additional information, please contact the undersigned. <br /> Sincerely, <br /> NewFields Mining Design&Technical Services Reviewed by: <br /> Jay Janney-Moore, P.E.
