Box 191 <br /> Victor,Colorado 80124 <br /> Attention: Laurin Colby <br /> Senior Metallurgist <br /> Re: RECORD OF CONSTRUCTION REPORT <br /> VLF2-Removal of the Crushed Ore Stockpile <br /> &Liner Integrity Verification Project <br /> Dear Mr. Colby, <br /> NewFields performed Construction Quality Assurance(CQA)activities during the removal of a crushed ore <br /> stockpile located within limits of the Squaw Gulch Valley Leach Facility (VLF2), and observing and <br /> documenting the pot holing activities in the Drain Cover Fill(DCF)to verify the integrity of the existing 80 <br /> mil LLDPE geomembrane liner. The stockpile removal and DCF pot holing activities occurred between <br /> November 27, 2018 and December 10, 2018. <br /> This letter serves as a record of construction report for the VLF2 Removal of Crushed Stockpile & Liner <br /> Integrity Verification Project. All construction activities were observed and verified to be in accordance <br /> with the approved Squaw Gulch VLF Technical Specifications revised and re-issued for construction on <br /> August 25, 2016. <br /> 1.0 REMOVAL OF THE CRUSHED ORE STOCKPILE <br /> Ames Construction (Ames)was contracted by Newmont's Cripple Creek and Victor Gold Mining Company <br /> (CC&V)to remove a temporary crushed ore stockpile placed with the VLF2 limits. The stockpile contained <br /> approximately 14,000 cubic yards of crushed ore and was located is an isolated area of the geomembrane <br /> lined pad situated north of the Mill platform and below the main access haul road into VLF2. <br /> Using CAT D6T LGP dozer, equipped with GPS, Ames pushed the crushed ore material downhill onto the <br /> 9960' lift. While using the CAT D6T dozer, Ames maintained a minimum of 2.5 feet of material in place <br /> above the geomembrane liner. The crushed ore were then loaded up into haul trucks and removed from <br /> the area. <br /> 2.0 LINER INTEGRITY VERIFICATION <br /> Once all the ore material was removed, the surveyor marked the location of 34 pot holes for the visual <br /> inspection of the geomembrane liner.
