1 | Page <br /> <br />Mine Site Inspection <br /> <br />Mine Name: Hayden Gulch Loadout <br />Operator: Hayden Gulch Terminal, Inc. <br />Permit ID#: # C-1992-081 (C-081-92 in the OSMRE I and E database) <br />Inspection Date: October 17, 2018 <br />Time: 8:30am – 12:00pm <br />Inspection Type: Complete Oversight Inspection <br />Weather Conditions: Dry, clear, and turning from cool to warm during the inspection. <br />Participants: <br /> <br />Dan MacKinnon: Office of Surface Mining Reclamation and Enforcement (OSMRE) <br />#554 <br />Tabetha Lynch: Division of Reclamation Mining and Safety (DRMS) <br />Miranda Kawcak: Hayden Gulch Terminal, Inc. <br /> <br />Overview <br />OSMRE conducted a complete oversight inspection with DRMS personnel and the operator’s <br />representative on October 17, 2018, which included: reclamation and its associated vegetation; a <br />records review; culverts; and ponds. Overall, this inspection found the sediment and surface <br />water control and management to be effective and functioning as designed; three maintenance <br />items were noted and are detailed below. No enforcement actions were issued as a result of this <br />inspection. <br /> <br />Inspection <br />Records Review. All but two records were current. Field inspections for the third quarter <br />impoundment report have been performed, but the report has not yet been written. The operator <br />stated that it will be available by November 1, 2018. The September 2018 DRMS inspection <br />report was also not available; the operator stated that they could not find the copy in house or <br />online and DRMS agreed to send the inspection report as an attachment to an email. We also <br />discussed the HG Loadout discharges and vegetation monitoring at the site; the operator stated <br />that there have been no discharges at either discharge point in the recent past, and that they did <br />not sample any vegetation this year in support of reclamation and / or bond release. The operator <br />stated the weeds had been sprayed on site in June and September of 2018. <br /> <br />Wetlands. There is a small area of the permit on the north side of Highway 40, and it includes <br />some wetlands. The operator contracted to have these wetlands evaluated for the U.S. Army <br />Corps of Engineers (USACE); the report is complete and will be sent to the USACE by <br />November 1, 2018. At the time of inspection, these wetlands were viewed from the old rail line; <br />other than the abundance of cattail, it was difficult to determine the overall biological <br />characteristics of these wetlands (Figure 1). <br /> <br />The Rail Line Intersection with Highway 40 and the Line Emanating North. Both the <br />intersection reclamation (removing the rail from the road and repaving, along with other removal <br />and reclamation adjacent to the road) and the final grading of the old rail line north of Highway <br />40 (to meet the grade of the hay fields adjacent to the permit) will likely happen the spring of <br />2019. This site was stable with no issues at the time of inspection.
