TECHNICAL REVISION NO. 5—Plant Modification <br /> HUKILL GULCH MILLSITE (DRMS Permit No. M-2009-076) <br /> Venture Resources, Inc. <br /> The facility's upper level will accommodate the ball mill, cyclone and pump equipment, and one of the <br /> gravity concentrators. The facility's lower level will accommodate a second gravity concentrator, as well <br /> as concentrate storage and lime storage. It is envisioned that any future reincorporation of a froth flotation <br /> circuit would likely occur on the lower level. Venture will also establish an office area within the <br /> confines of the new structure. <br /> Pole mounted electrical service is already in-place at the process plant area, extending to the existing pad- <br /> mounted 480V transformer and electrical distribution panel proximal to the crusher trailer unit. Service is <br /> to be extended from that point to the new structure via a buried segment (approximately 75 ft. in length) <br /> extending to and entering the southeast end of the structure. It is anticipated that the buried extension will <br /> consist of 3/0 AWG cable. <br /> Raw (process) water supply to the new structure will be from a tank installation situated approximately <br /> 100-ft. upgradient and to the northwest of the completed structure. Raw water will continue to be brought <br /> in to the site via truck (per the as-approved permit). Venture will continue to recycle process water <br /> (during active processing) through the circuit, employing two discrete lines (one for raw process water <br /> going to the mill, and one for recycle process water going back from the mill) enclosed within an HDPE <br /> secondary containment line, thus ensuring secondary containment over the full length of the service <br /> line(s). <br /> Venture will ensure that the tank installation is incorporated into the facility's Spill Prevention Control <br /> and Countermeasures (SPCC) Plan and Stormwater Management Plan (SWMP), respectively, and as <br /> such, it will be equipped with an earthen berm surround that will function as a secondary containment <br /> structure that is volumetrically sized to contain 150% of the total storage volume. Venture will maintain <br /> portable pump capability onsite that can be rapidly deployed to evacuate the contents of the secondary <br /> containment structure, if necessary. <br /> Permit-Related Considerations: The Hukill Gulch Mill site's tailing storage facility was the subject of a <br /> 2017 "Corrective Action" carried out to accomplish repairs to the Environmental Protection Facility <br /> (EPF) triple liner system. The subject"Corrective Action" was documented and certified by a Colorado <br /> Registered Professional Engineer and the "certification" accepted by the Colorado Division of <br /> Reclamation, Mining and Safety on November 2, 2017. Accordingly, Permit No. M2009-076 is, to the <br /> best of our knowledge, considered to have fully active status, subject to the approved permit conditions <br /> and stipulations. However,no mining or milling operations have been carried out subsequent to receipt of <br /> the certification acceptance; routine site monitoring and security, site cleanup, and equipment <br /> maintenance activities have been carried out and are ongoing. No activities relative to the proposed plant <br /> upgrade(that is the subject of this Technical Revision No. 5)will commence until such time that Division <br /> authorization is received. <br /> All existing environmental protection facility components associated with the process plant area will <br /> remain functional and relevant to the proposed plant upgrade and process plant area modifications. <br /> 4 <br />
