Exhibit F—list of other permits and Licenses Required <br /> As required by the application instructions for the Limited Impact Operation (110(1)) Reclamation <br /> Permit Application Package "Compliance with Other Laws" section,the following agencies have been <br /> contacted to inquire about additional permitting or compliance requirements. Information pertaining to <br /> additional requirements is noted per agency below; <br /> 1. The Colorado State Historical Preservation Office. <br /> In Colorado the State Historical Preservation Office (SHPO) is housed within the Office of <br /> Archaeology and Historical Preservation (OAHP). <br /> A call was placed by Joe Lovett to the OAHP on 6/14/2018 and was directed to leave a message <br /> for Stephanie Boktar(information management team—303-866-5216).As of the date of the <br /> submission of this application the OAHP had not returned the phone call. <br /> Please note that although there is some old mining equipment located onsite(wood feed box <br /> structure and trommels)the active mining area is approximately 150 feet away and not in the <br /> immediate area of the trommels. It is not intended to disturb the trommels in any way. <br /> There are no other structures, etc. in the area that would be considered "historical'. <br /> 2. Colorado Division of Water Resources. <br /> Information from the Colorado Division of Water Resources was requested by Joe Lovett for <br /> any requirements concerning via their website (AskDWR Information Request).This was sent to <br /> the DWR on 6/13/2018. <br /> On 6/20/2018 a response was received from John Van Oort(Division 2—River Operation <br /> Coordinator—see also attached e-mail chain)which stated that a Substitute Water Supply Plan <br /> (SWSP)would be needed to replace water caused by evaporation from the settling pond. We <br /> have asked for clarification on threshold for when a SWSP is required as the settling pond for <br /> the El Rancho Bondo operation is only 5 to 10 feet in diameter and is present only a few hours <br /> every week. <br /> As of the date of the submission of this application the Division of Water Resources had not <br /> responded to the above questions. <br /> We will follow up with a submission of the SWSP(if required)to the State Engineer. <br /> 3. CDPHE Water Quality Control Division. <br /> An e-mail requesting information concerning water quality permitting requirements from the <br /> Colorado Department of Public Health & Environment Water Quality Control Division was sent <br /> to the CDPHE website cdphe.commentswqcdPstate.co.us on June 13, 2018. Please see attached <br /> copy. <br />
