3.2 Spill Response and Notification <br /> 3.2.1 Spill Reporting <br /> In the event of a spill, a plant operator shall notify the Plant Manager or Spill Prevention <br /> Designee and complete a written Spill Notification Form (see Appendix 6). This form details <br /> the time, material, and quantity of oil released. <br /> Spills ultimately reaching a watercourse in harmful quantities as defined by 40 CFR 110.3 <br /> are required to be reported under 40 CFR 110.10. The regulatory definition of harmful <br /> quantities is a discharge which: <br /> • Violates applicable water quality standards, or <br /> • Causes a film or sheen upon or discoloration of the surface of the water or adjoining <br /> shorelines or causes a sludge or emulsion to be deposited beneath the surface of the <br /> water or upon adjoining shorelines." <br /> The Plant Manager or Spill Prevention Designee shall notify the agencies listed in Section <br /> 3.2.2 by telephone about the size, nature, and impact on the waters. <br /> As noted in Section 1.0 (Introduction), the SPCC regulations require that the SPCC plan be <br /> submitted to the U.S. EPA regional administrator and to the appropriate state agency within 60 <br /> days, along with other information as outlined in paragraph 112.4, if either of the following <br /> occurs: <br /> • the facility discharges more than 1,000 gallons of oil in a single event, or <br /> • the facility experiences two discharges (over 42 gallons) within any 12-month period <br /> A spill notification form is presented in Appendix 6. The report must be submitted to the EPA <br /> Regional Administrator within 60 days from the time the spill occurs: <br /> The Plant Manager or Spill Prevention Designee is responsible for completing and mailing the <br /> spill notification form and other information as may be required. <br /> a. Name of the person reporting the incident <br /> b. Name and location of the installation <br /> c. Phone number where the person reporting the spill can be reached <br /> d. Date, time, and location of the incident <br /> e. A brief description of the incident, nature of the materials or wastes involved, extent of <br /> any injuries, and possible hazards to human health or the environment <br /> f. The estimated quantity of the materials or wastes spilled, and <br /> g. The extent of contamination of land, waters, or air, if known. <br /> During an emergency, the Plant Manager or Spill Prevention Designee is trained to undertake <br /> and must take all reasonable measures necessary to ensure that fire, explosion, emission, or <br /> discharge do not occur, reoccur, or spread to other materials or wastes at the installation. <br /> These measures shall include where applicable, stopping manufacturing processes and <br /> operations, collecting and containing released materials or wastes, and removing or isolating <br /> containers. <br /> Portable Gencor Asphalt Plant Page 10 <br /> SPCC Plan August 31, 2005 <br />
