Recently the operator has noticed
unauthorized access to the site. Impediments have been placed on the road to limit the access to the eastern
section of the facilities area. If full access to the haul road was needed the impediments could be moved quickly.
Under the circumstances, the division finds this acceptable. Unauthorized access had been through Light Use
Road #2. The operator has placed restrictions in front of the gate to the road way. If access to this road was
needed, then the operator will take the necessary action to remove the restrictions. The Division finds that these
actions are acceptable.

SLIDES and DAMAGE - Rule 4.12:
There was no new rock fall observed at the site. The rock fall protection fencing at the portals remains in
place and was functioning as designed.

SUPPORT FACI LITIES - Rule 4.04:
Chuck Silengo was on-site during the inspection working on organizing and cleaning the pad around the
mine office. Due to recent break ins at the site, restrictions on access to the upper portion of the haul road and
Light Use Road #2 have been put in place. The restrictions appeared to have worked since their implementation.
There has been no damage to the hydrologic controls, material storage areas, sumps or culverts. The Division
finds that these actions are acceptable and do not impact the functions of the hydrologic controls or facilities.

TOPSOIL – Rule 4.06
Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4:
The Topsoil pile was stable and well vegetated (Photo 5). There were no erosional concerns identified.
The perimeter ditch to the pile was in good condition and dry at the time of the inspection. The topsoil marker
sign was in place on top of the pile and easily located.
