o Transit Mix Concrete Co. <br /> 444 E.Costilla Street,Colorado Springs,CO 80903 Office: (719)475-0700 Fax: (719)475-0226 <br /> • Audible blast signals, originating from a vehicle mounted siren, will be used <br /> according to the following schedule: <br /> 0 2 minutes before the blast—Three short 15 second audible siren blasts. <br /> 0 30 seconds before blast—Two short 15 second audible siren blasts. <br /> • Once all personnel are accounted for, all blast guards are in place, and the blast <br /> area has been confirmed to be clear, the blaster in charge will initiate the blast. <br /> Post Blast Inspection Procedures <br /> After the blast is completed the blaster in charge will re-enter the blast area and inspect <br /> the entire blast muck pile to make sure all holes were detonated. No one will be allowed <br /> back into the mining, plant, or shop areas until the blaster in charge has inspected the <br /> blast and confirmed that it is safe to resume work. At this time: <br /> • The All Clear audible signal will sound—One long 30 second audible siren blast. <br /> • Blast guards will release their blocks at the defined Blast Area within the quarry <br /> and guards securing the additional safety perimeter, including Little Turkey Creek <br /> Road, within the Hitch Rack Ranch property will be removed. <br /> • Personnel are allowed back to work. <br /> • Customers are allowed back to the product loading area. <br /> • Business returns to normal. <br /> Misfire Procedures <br /> A misfire is the complete or partial failure of a blast hole to detonate as planned. With <br /> advancements in product quality and blast technology, misfires are extremely rare, <br /> however in the event of a misfire blaster in charge will: <br /> • Not order the all clear signal and not release the blast area for return to work and <br /> notify management that there is suspicion of a misfire. <br /> • Wait the MSHA required 15 minutes to begin a complete and more thorough <br /> investigation of the extent of and cause of the misfire. <br /> o Depending on the location of the blast and the hazard level the blaster in <br /> charge determines the misfire presents, the blaster in charge, at his <br /> discretion, can release only those areas not in the designated blast area <br /> during the required waiting period. This area would include the area <br /> outside of the designated blast area and within the additional safety <br /> perimeter as well as other facilities on the site outside the designated blast <br /> area. <br /> • If the blaster in charge's investigation reveals the misfired hole or holes can be <br /> safely reblasted, the newly designated blast area and the additional safety <br /> perimeter will be cleared again, blast guards reestablished, and the blast warning <br /> procedure will be reinitiated. After the misfire is reblasted and the blaster in <br /> charge completes his post blast inspection, he will order that the all clear signal be <br /> sounded. <br /> 13 <br />
