iii. Haul contaminant debris offsite <br />iv. Break and bury concrete slab onsite <br />V. SW -33 Compliance Monitoring station — remove when no longer <br />needed <br />vi. Grade site and spread growth medium as available <br />vii. Revegetation Seeding <br />Sediment Pond, Sediment Disposal <br />i. Pond to remain as permanent feature <br />ii. Ensure Dam Structure intact, spillway is competent, grout fill all pipes <br />iii. Sediment Removal - dispose of in TCC Dump under sericite cap <br />j. Sandstone Quarry adjacent to Sediment Pond Dam <br />i. Grade slope — push down to more stable gradient <br />ii. Cut and remove trees above highwall <br />iii. Collect excess riprap material for use onsite <br />iv. Scatter slash from cut trees onto new slope <br />k. Millsite and Stormwater Pond <br />i. Millsite was affected but all grading and revegetation is adequate <br />ii. Stormwater pond (across road) needs riprap at drainage inlet below <br />road <br />iii. Revegetation - lodgepole and grass on mill and pond site are adequate <br />iv. After riprap placement, request release of these areas <br />I. Limestone Quarry <br />i. Quarry is presently reclaimed, but can be redisturbed if further riprap <br />is needed on site <br />ii. For generating more riprap - cut trees from above highwall, push down <br />rocky material, leave rocky highwall at steep, stable gradient <br />iii. Scatter tree slash and broadcast seed on slope and flat below slope <br />m. Tailings Pond and Tailings Pipeline Corridor, in Hale Gulch <br />i. No tailings dam was constructed, no pipeline or road ever constructed <br />ii. Check corridor and dam/pond sites to ensure no disturbance <br />iii. Tailings dam site access road has been barricaded <br />iv. Request release of all of these areas <br />n. Piezometers, Inclinometers, Injection Wells <br />i. Remove all features not required; install new features as needed <br />ii. Removal: cut off well pipe extending above ground at wellheads <br />iii. Remove surface piping and temporary structures <br />iv. Seal and document the wellhead abandonment per State Regulations <br />V. Finish grade and broadcast seed <br />
