• Approximately 15 plus cubic yards of soil was excavated and transported to lined portions of Arequa <br /> Gulch Valley Leach Facility. <br /> • The over-excavated areas were filled in with clean fill and gravel. <br /> b. Results of the Operator's investieation <br /> The root cause of the release was an oversight in starting the blast furnace at AGADR. Maintenance was <br /> performed on the sump pumps in the refinery earlier in the week and the pumps were left in the off <br /> position. Refinery personnel started the blast furnace and left the refinery. When refinery personnel <br /> returned, they noticed the release and immediately turned on the sump pumps.
