Page 2 <br /> Mr. Eric Scott <br /> November 27, 2017 <br /> EXHIBIT E-Reclamation Plan(Rule 6.4.5): <br /> (2)(e)A plan or schedule indicating how and when reclamation will be Implemented... -The phasing is <br /> show on the Reclamation Plan Sheet. <br /> The Division will assume at this time that all exposed groundwater bodies will be lined with a slurry wail for <br /> planning and bonding purposes.-The miner has provide a bond for this and will update the current <br /> bond as necessary. <br /> The permittee will need to commit to submitting a technical revision detailing the proposed construction <br /> specifications and configuration prior to installation of any additional slurry walls on-site. -Once approval <br /> of this amendment is complete the miner will provide a TR with preliminary design of the proposed <br /> slurry walls. <br /> The permittee will need to commit to submitting a technical revision to the permit if the final reclamation plan <br /> differs from the configuration currently shown in AM02, or if existing exposed groundwater areas will not be <br /> isolated from the alluvial aquifer with a slurry waN approved by the SEO. -Once approval of this <br /> amendment is complete the miner will provide a TR showing the prosed final reclamation plan <br /> updates. <br /> EXHIBIT F-Reclamation Plan Map(Rule 6.4.6). <br /> Please indicate acreages referred to in Exhibit L (scarified ground, topsoil replacement, seeding)on figure 4 <br /> Rec Plan map. Please show as-built slurry walls and'Proposed"slurry wall locations/configurations used to <br /> calculate footage costs for Exhibit L. -The acreages are shown on Sheet 4. <br /> EXHIBIT L -Reclamation Costs(Rule 6.4.12). (1)All information necessary to calculate the costs of <br /> reclamation must be submitted and broken down into the various major phases of reclamation. You must <br /> provide sufficient information to calculate the cost ofreclamation that would be incurred by the state. This <br /> should also include cost for installation of appropriate flood control structures for reservoirs in the flood plain. <br /> -The costs have been updated. The spillways have been included as well in the cost. The cubic <br /> yard cost was taken from a recent reservoir project. Each spillway allows for a fill rate that will allow <br /> the ponds to fill to help prevent washout of the slurrywall. A length and cubic yard of spillway has <br /> been provided on the reclamation plan sheet. Once the flooding has passed the spillways are large <br /> enough to release water that will spread out to limit erosional velocities. <br /> What is the source of the unit cost for slurry wall installation utilized?-A recent bid from another deeper <br /> slurry wall just east of the site. See attached bid. <br /> EXHIBITS-Permanent Man-Made Structures(Rule 6.4.19). <br /> Please note that roadways and above-ground or underground utilities(if present)within 200 feet of the <br /> proposed affected area are considered permanent man-made structures. In accordance with Rule 6.4.19, <br /> when mining operations will adversely affect the stability of any significant, valuable and permanent man- <br /> made structure located within 200 feet of the affected area, the applicant may either, <br /> a.) Provide a notarized agreement between the applicant and the person(s)having an interest in the <br /> structure, that the applicant is to provide compensation for any damage to the structure;or-All owners of <br /> property within 200'of site boundary have been sent certified mail containing such agreements. <br /> Verification is attached to this letter. <br /> b.) Where such an agreement cannot be reached, the applicant shall provide an appropriate engineering. <br /> evaluation that demonstrates that such structure shall not be damaged by activities occurring at the mining <br />
