("VRI") attaches hereto the Corrective Action Completion and <br /> Recertification Report as pertaining to the required repairs to the Environmental Protection Facility <br /> (EPF) composite liner system at the VRI Hukill Gulch Millsite. <br /> The Corrective Action Program was carried out in a manner generally consistent with the Division- <br /> approved Technical Revision Nos. 3 and 4, respectively. As a component of Technical Revision <br /> No. 4, VRI is required to provide the attached completion report and recertification upon <br /> conclusion of the corrective action activities. As required, the Corrective Action Program was <br /> carried out under the direction of an independent, third-party Colorado Registered Professional <br /> Engineer (James M. Beck, P.E.; Colorado Registered Engineer No. 25393) of J. M. Beck & <br /> Associates Mining Consultants. Mr. Beck has also prepared the Completion Report and <br /> Recertification document, to include provision of the liner recertification seal. <br /> There are four (4) items that we specifically bring to your attention, each of which is individually <br /> addressed in greater detail within the body of the attached Completion Report and Recertification <br /> document, but for which a formal Division response is respectfully requested: <br /> 1) Division concurrence and written authorization to allow VRI to return the temporarily <br /> containerized liquids (that were removed from the impoundment) back into the lined <br /> impoundment as the method of final disposition; <br /> 2) Division concurrence and written acknowledgement that the Corrective Action Program <br /> has been satisfactorily completed and re-certification of the composite liner system <br /> accepted; <br />
