Access Portal Collapse Repair and Equipment and Stope Rock Removal Work Plan <br />March 2, 2012 <br />May Day No. 1 <br />3.0 CONSTRUCTION IMPLEMENTATION PLAN <br />This section includes a general description of the construction implementation for Phase 1 (Tasks <br />1 to 4), discussion of the construction documentation and reporting procedures, the construction <br />quality control and assurance guidelines and the project closeout and final construction <br />completion report. More details on Phase 2 construction implementation and closeout will be <br />provided with the Phase 2 design document. <br />Construction implementation for Tasks I through 4 is anticipated to begin as soon as possible <br />following approval of this work plan by DRMS.It 1 weather estimated <br />that approximately 3 to 4 weeks <br />may be required to complete Tasks 1 to 4 depending <br />Construction will begin with installation of necessary runoff and erosion BMP controls discussed <br />above. Initial work will also include installation of safety features and controls. The steel portal <br />shield will be installed some distance into the existing access portal after it has been re -opened, <br />as specified by the site engineer. <br />Following completion of these controls, excavation of collapse materials will begin with <br />transport to the May Day No. 2 bench area. Removal of buried crushers and other equipment <br />will follow next along with removal and replacement of damaged steel sets within the Access <br />Portal. Initial stabilization procedures for the area outside the adit will be performed as <br />necessary to achieve adequate stability until the Phase 2 long-term stabilization procedures are <br />performed. <br />3.1 Documentation and Construction Reporting <br />Prior to Phase 1 work, a pre -construction meeting will be held with the construction contractor, <br />owner and engineer. This pre -construction meeting will cover topics such as roles and <br />responsibilities, communication, health and safety, procedures for field and design changes <br />during construction and schedule. Minutes of the pre -construction meeting will be issued to <br />attendees within three work days following the meeting. <br />During construction daily reports generated by the contractor will be reviewed by the field <br />inspection technician and engineer of record on a daily basis. The technician will prepare daily <br />construction quality assurance (CQA) reports describing his/her observations and actions taken <br />to correct deficiencies in the work. Field test results and the contractor's daily reports will be <br />included as attachments. The technician's CQA reports will be forwarded to the geotechnical <br />engineer for review and action (if any is required).
