MAY DAY IDAHO MINE COMPLEX <br />M -1981-185,112d-2 AMENDMENT (AM -2) APPLICATION <br />SUNRISE MINING, LLC <br />EXHIBIT D <br />Mining Plan <br />Rule 6.4.4 <br />The May Day Idaho Mine Complex permit area is approximately 295.79 acres of which approximately <br />45.7 acres will be within the affected mine area. The mine was historically opened in the early 1900s and <br />closed in 1942. Historical May Day Mine activities occurred at the May Day No. 1, May Day No. 2, <br />Brooklyn and the surface "Glory Hole" mine workings (see Figure D-1). The Idaho Mine includes <br />multiple mine development areas: Idaho 1, 2, 3 and 4, Grizzly, May Day No. 3 (Lamb), Derby and May <br />Day No. 4. In the early 1980s, the mine was reopened and has operated periodically over the last 30 years. <br />In 2007, the prior Operator commenced redevelopment activities with a conditional mine permit being <br />approved on December 12, 2011. The prior Operator's conditional mine permit was suspended on <br />November 26, 2013 and reinstated on January 9, 2017. On February 9. 2017, Sunrise Mining, LLC <br />("Sunrise") was approved as the permitted operator of the May Day Idaho Mine Complex. <br />Since 2011, to address certain deficiencies attached to the "conditional" permit the following tasks were <br />completed: <br />• New Access Road was stabilized and reclaimed; <br />• May Day No. 1 portal was stabilized and reclaimed (see Attachment D-1); <br />• Chief portal and the access road immediately above were stabilized; <br />• Little Deadwood Gulch channel sections at the May Day No. 2 and Chief were reclaimed; <br />• Roads were maintained; <br />• Erosion and sedimentation controls were installed; <br />• Water quality characterization, monitoring and reporting was initiated. <br />Over the next 2-5 years, Sunrise will be focused on the: (i) removal and processing of surface ore <br />stockpiles located within the Idaho designated affected area, (ii) removal of underground stockpiles <br />located on the floor of the May Day No. I stope and (iii) development and mining of the area referred to <br />as the Lamb Raise May Day No. 1(see Figure D-2). Each of these three mine activities are outlined in <br />greater detail herein. <br />The Idaho ore stockpiles were established in early 1980, and the May Day No. 1 underground ore <br />stockpile is believed to have been formed prior to 1935.
