• Approximately 4 cubic yards of slurry and soil were excavated with a front end loader and dumped
back into the float concentrate thickener secondary containment, the slurry and soil were washed into
the sump, thus being pumped back into the mill circuit.
• The over-excavated area was filled in with clean fill and gravel (see photo 4).
b. Any known or adverse impacts to human health, property or the environment
No impacts to human health or the environment were observed or are expected to occur due to the swift
clean up.
c. CC&V Contact
For any further information regarding this event please contact;
Meg Burt
Senior Environmental Manager
PO Box 191,
100 N 3rd St,
Victor, Colorado, 80860
Work: 719-689-4055
Cell: 775-397-3571
Fax: 719-689-3254
d. Monitoring and analyses that are necessary to evaluate the situation and corrective actions
Sampling is not necessary because the chemical composition of the slurry is well known, furthermore due
to the thick nature and color of the slurry CC&V can ensure that all of the slurry was removed.
e. Results of the Operator's investigation
The root cause of the spill was a failure of the 34A pump housing and the partially open service doors. If
the doors were closed the slurry spray from the pump would have remained within secondary
containment. Increased training will be rolled out to the appropriate personal about keeping the doors
closed.
Should you require further information please do not hesitate to contact Tyler O'Donnell at 719.689.4056 or
`II ler.O'Donnell conewtuotit.cotn or myself at 719.689.4055 or Meg.Burt(a?newmont.com.
Sincerely,
Meg Burt
Senior Environmental Manager
Cripple Creek and Victor Gold Mine
MB/tvo
ec: Tim Cazier—DRMS
Wally Erickson-DRMS
File
