SEP 18 2017 <br /> Environmental Protection Specialist DMSION OF RECLAMATION <br /> Colorado Department of Natural Resources MINING AND SAFETY <br /> Division of Reclamation,Mining and Safety <br /> Office of Mined Land Reclamation <br /> 1313 Sherman Street,Room 215 <br /> Denver, Colorado 80203 <br /> Re: Permit No.M-1980-244; Cripple Creek& Victor Gold Mining Company("CC&V"); Cresson Project <br /> -Written notification—Float Concentrate Slurry Spill—September 11,2017 <br /> Dear Mr. Cazier: <br /> In accordance with Rule 8, Section 8.2.3 of the Mineral Rules And Regulations of the Colorado Mined Land <br /> Reclamation Board For Hard Rock, Metal, And Designated Mining Operations (Rules), the Cripple Creek and <br /> Victor Gold Mining Company (CC&V) hereby provides the Division of Reclamation, Mining and Safety <br /> (Division) with a formal report regarding the Concentrate slurry spill which occurred at the Concentrate thickener <br /> at the High Grade Mill(HGM), which occurred late morning September 11, 2017. <br /> The following points address the requirements of Rule 8, section 8.2.3: <br /> a. Actions taken to respond to and correct the emergency situation or condition <br /> • The spill was observed by a mill operator preforming a daily walk around inspection. The spill <br /> resulted from a small hole in the bowl of the 34A pump (see photo 1). The slurry was spraying from <br /> the pump to just outside of the concrete secondary containment stem wall (photo 2). Very little slurry <br /> spilled into the secondary containment. The spill occurred because the service doors to the area below <br /> the thickener were left partially open, thus allowing the slurry to spray out of containment. CC&V <br /> estimates that approximately 25 gallons of slurry were spilled outside of the float concentrate <br /> thickener secondary containment. <br /> • The 34A slurry pump was immediately shut off and the valves feeding the pump were closed. The <br /> redundant 34B slurry pump was turned on, and the process returned to normal in a matter of minutes. <br /> • A front end loader was used to recover the float concentrate slurry and the contaminated soil. The <br /> concentrate slurry is very thick, thus not allowing it to infiltrate more than an inch or two. The soil <br /> was removed using an `over-excavation' approach. Depth of recovery ranged from 6 inches to 18 <br /> inches (see photo 3). Slurry was recovered from the first few inches of soil; the underlying soil was <br /> not wet;therefore, CC&V can ensure that all contaminated soil was removed. <br />
