1. As required by Rule 1.6.2,proof of publication was submitted from Pikes Peak Courier. Please <br /> see attached letter from the publisher. <br /> 2. As required by Rule 1.6.2,proof of the notice to all owners of surface and mineral rights,etc. <br /> USFS is the boundary. <br /> 3. Letters received by the Division from Teller County(April 25,2017),copies of everything have <br /> been sent to Teller County Community Development Services. The Planning Department will be <br /> sent an annual report and map as required by the State of Colorado Division of Reclamation, <br /> Mining and Safety and any updated USFS Plan of operation. Division of Water Resources(April <br /> 26,2017),per all of the plans submitted to you prior,there are no changes and no water worries <br /> should arise; therefore meeting the requirements of 110(2). Colorado Parks and Wildlife(May <br /> 3,2017),has been received, noted,and filed. History Colorado(May 5,2017)has been <br /> acknowledged and filed. <br /> 4. All maps submitted comply with Rule 6.2.1(2). <br /> S. Regarding EXHIBIT B-Site Description(Rule6.3.2), please see attached letter from Christopher <br /> Smith of USDA-NRCS describing native vegetation. <br /> 6. Please see Web Soil Survey-National Cooperative Soil Survey provided by NRCS. <br /> 7. Regarding EXHIBIT B-Site Description(Rule 6.3.2),please see attached letter from NRCS stating <br /> depth of ground water. <br /> 8. Re: EXHIBIT C-Mining Plan-We will screen on site with an off horizontal 3/8" mesh screen for <br /> hard separation of the dirt and rock. No water is used. <br /> 9. We confirm we will have one dig site 20'x 60'and up to 15'deep open at one time and this dig <br /> site will be back filled before moving onto the second site. We also confirm we will not have <br /> more than two acres disturbed at one time. <br /> 10. We confirm that all dig sites and mining related disturbances will occur in the seven proposed <br /> permit boundary areas. <br /> 11. Storm water from the mining operations on Amazonite#4,#6,and#8 will be controlled by logs <br /> and sticks,an edge will go around the site to keep waste water in and a platform will be built <br /> into dig site. Vegetation has grown to dig site so it will also assist in erosion control. Also, <br /> please see attached letter from USDA-NRCS,#11,regarding the storm water. <br /> 12. We will reclaim with 16 Ponderosa Pine(pinups Ponderosa),2-3 years old,with root ball and a <br /> height of 12"to 24". Also,please see letter from USDA-NRCS. <br /> 13. Dig sites will not occur on lands steeper than 3H;1V. <br /> 14. The older seeding mixture was used in entirety to reclaim Maconite#VII. New seed mixture <br /> (Montane Seed Mixture), required by Teller-Park Soil Conservation District,will be used from <br /> this day forward. <br /> 15. PLS lbs./acre seeding rate is 5-10 lbs. <br /> 16. No sage brush will be used as per USDA-NRCS,Teller County Soil Conservation District states; <br /> that only trees and grass mixture(Montane)is adequate. <br /> 17. No manure will be used for reclamation. <br /> 18. Pertaining to EXHIBIT E permit boundary within Amazonite#7 that has a 12'x 40'access road; <br /> we will reclaim from the forestry road back to the claim adding topsoil,trees,and grass as per <br /> USDA-NRCS. <br /> 19. In accordance with Rule 3.1.5(31 grading will be done at max.3:1 with cofferdams,platforms, <br /> tree stumps,and twigs to control water flow to prevent erosion. <br /> 20. We confirm that the yellow areas identified on the Mine Plan Maps, referred as"Bonded Dig <br /> Areas",are extents of the proposed permit boundary. <br /> 21. Please see revised maps. <br />
