GW-3A well installation documentation <br /> indicates that recirculation was lost during installation and that the well was installed"in the blind", <br /> making the well more susceptible to performance issues. Field observations during sampling of GW-3A <br /> have noted turbid water, filter pack sand on the sampling equipment, and algae growth in the casing. A <br /> damaged well could result in anomalous metal detections as solid materials enter the well bore and react <br /> with the water contained there-in. OSMI plans to reinstall the well using the attached stamped design as a <br /> part of the Q3 2017 maintenance program. <br /> 31Page <br />
